Got a liver report with SGPT marked high? Here is what the number means, what can raise it and when to get help.

An SGPT test measures alanine aminotransferase (ALT), an enzyme found mostly inside your liver cells. When those cells are hurt, ALT leaks into your blood and the number goes up. The 2017 American College of Gastroenterology (ACG) guideline puts a healthy SGPT at about 29 to 33 U/L in men and 19 to 25 U/L in women.1
A raised SGPT is common. It often points to liver cell injury but does not show the cause. Fatty liver, alcohol, hepatitis and some medicines are common reasons.
Key Takeaways
- The SGPT test measures ALT, a liver enzyme. It often points to liver cell injury, not how well your liver works.
- The ACG guideline suggests a healthy SGPT of about 29 to 33 U/L in men and 19 to 25 U/L in women. Many lab limits are higher.
- Fatty liver is one of the most common reasons for a mild rise. Pooled Indian studies estimate it in roughly 4 in 10 adults, with wide variation.
- Yellow eyes, confusion or a paracetamol overdose need urgent care, whatever the number.
What Is an SGPT Test?
SGPT Full Form and Its New Name
SGPT stands for serum glutamic pyruvic transaminase. Most labs now call it ALT, short for alanine aminotransferase. Both names mean the same enzyme.2
The kidneys, heart and muscles hold small amounts, but the liver holds by far the most.3
Why Doctors Order This Test
SGPT is usually part of a liver function test (LFT) or a routine checkup. Doctors also order it for symptoms like tiredness or yellow eyes, and for people with diabetes, obesity, alcohol use or past hepatitis.2
The test is a simple blood draw from your arm. Follow your lab’s fasting instructions.2
What Is the SGPT Normal Range?
A 2002 Italian study tested 6,835 first-time blood donors. Using only the low-risk donors, it set new upper limits of 30 U/L for men and 19 U/L for women.4
The 2017 ACG guideline suggests 29 to 33 U/L for men and 19 to 25 U/L for women. It based this on healthy people and on links between higher ALT and liver deaths.1 The 2023 American Association for the Study of Liver Diseases (AASLD) guidance uses the same range and notes that lab limits are often higher.5
Why Your Lab Report Range May Look Different
Each lab sets its own range. An SGPT of 38 U/L may look normal on your report, yet it is above the ACG range for both men and women. Your doctor reads it with your symptoms and other tests.
Age, sex, body weight, some medicines, supplements, intense exercise and even your menstrual cycle can shift the number.2
What Does a High SGPT Mean?
A raised SGPT often points to liver cell injury, but it does not reveal the cause by itself. Intense exercise and some medicines can also affect the result.21
Common Causes, Including Fatty Liver
- Fatty liver: A very common cause in people with no symptoms.
- Alcohol: Regular drinking can injure liver cells over time.
- Viral hepatitis: Long-term hepatitis B or C, or a new hepatitis infection.
- Medicines and herbs: Some painkillers, TB medicines and herbal supplements. Rarer causes include autoimmune hepatitis and iron overload.3
A 2022 Indian review pooled 54 adult datasets (over 23,000 people). About 38.6% had fatty liver. The figure was 28.2% in community studies and 40.8% in hospital studies. In high-risk groups such as people with obesity or diabetes, it was 52.8%. Results varied a lot between studies, so these are rough estimates, not an exact figure for all of India.7
If your ultrasound shows grade 1, grade 2 or grade 3 fatty liver, it may explain a mild rise but does not prove it.
In 2023, global liver societies renamed non-alcoholic fatty liver disease (NAFLD) as metabolic dysfunction-associated steatotic liver disease (MASLD).6 MASLD means fat build-up in the liver along with at least one cardiometabolic risk factor, such as obesity, type 2 diabetes or abnormal blood fats. Doctors also consider alcohol use and other causes. Your HbA1c level is often checked next.
Mild, Moderate and Severe Rise Explained
ACG grades a raised SGPT by how many times it is above the upper limit of normal (ULN). ULN means your own lab’s upper limit. The examples below use 33 U/L only to show the math.1 The tests listed are examples, not a standard plan. Your doctor picks what fits you.
| Grade | Times Above ULN | Example SGPT (U/L) | Tests Your Doctor May Consider |
|---|---|---|---|
| Borderline | Less than 2x | 34 to 65 | A repeat test and questions about alcohol, medicines and weight |
| Mild | 2x to 5x | 66 to 165 | Possibly hepatitis B and C tests, iron studies or an ultrasound |
| Moderate | 5x to 15x | 166 to 495 | Possibly tests for a new hepatitis A, B or C infection as well |
| Severe | More than 15x | Above 495 | Prompt checks for sudden liver injury, such as paracetamol harm or low blood flow to the liver. ACG calls values above 10,000 U/L massive |

Moderate, severe or massive rises need immediate medical evaluation, says the ACG guideline.1 Do not wait for a routine repeat test if the level is very high or you feel unwell. Signs of acute liver failure, such as confusion with liver injury, need immediate care whatever the SGPT level.
When Is SGPT at a Danger Level?
There is no single danger number. Values above 1,000 U/L usually mean sudden injury. A 2023 review of seven studies with 1,608 patients found the top cause was low blood flow to the liver (51%). Viral hepatitis (13.1%) and drug or toxin injury (13%) came next.8
Smaller rises that last for months still matter. ACG links a raised ALT with higher liver-related deaths over time.1
When To See A Doctor
Book a visit with your doctor if:
- Your SGPT is above your lab range on a repeat test.
- You feel tired or weak for weeks, lose your appetite or often feel sick.
- You notice dark urine, pale stools, frequent itching or belly pain.
- You have diabetes, obesity or drink alcohol regularly and have not had your liver checked.2
Keep in Mind
Seek emergency care now, whatever your SGPT number, for:
- Confusion or extreme sleepiness: This can be a sign of serious liver trouble.
- Vomiting blood or a swollen, painful belly: These can mean bleeding or fluid build-up.
- A suspected paracetamol overdose: Go now, even if the person feels well.
In India, call 112 for any emergency. Many states also use 108 for ambulances.
Yellow eyes or skin (jaundice) also need urgent medical assessment. See jaundice symptoms.
What Does Low SGPT Mean?
Low SGPT is uncommon and rarely a worry. Possible reasons include vitamin B6 deficiency and chronic kidney disease.2 ALT needs a form of vitamin B6 to work.3 A kidney function test (KFT) may follow if needed.
SGPT vs SGOT: What Is the Difference?
SGOT is another liver enzyme, now called AST (aspartate aminotransferase). Doctors read both together.
| Feature | SGPT (ALT) | SGOT (AST) |
|---|---|---|
| Full name | Alanine aminotransferase | Aspartate aminotransferase |
| Main location | Mostly liver | Liver, muscles, heart and other organs |
| How liver specific | More specific | Less specific |
| Half-life in blood | About 47 hours | About 18 hours |
| Pattern clue | Usually higher than AST in fatty liver and viral hepatitis | AST more than twice ALT points towards alcohol-related injury |
Sources: ACG 2017, MedlinePlus 2024, StatPearls 2024. The AST to ALT ratio is a clue, not a diagnosis.
How to Read Your SGPT Test Report
- Check the unit: Labs write U/L or IU/L. For SGPT they mean the same.
- Check your lab’s range: The high flag uses that lab’s own limit.
- Read the other values: SGOT, alkaline phosphatase and bilirubin matter too.
- Think about the last few days: Hard exercise, new medicines, supplements or alcohol can raise it. Tell your doctor.2
- Look at the trend: Is it rising or falling?
How to Lower High SGPT
The plan depends on the cause. No detox drink is proven.
- Confirm the result: For a mild rise with no symptoms, the ACG guideline suggests repeating the liver tests first.1
- Lose extra weight if you have fatty liver: The 2023 AASLD guidance says losing 3% to 5% of body weight reduces liver fat. More than 10% is usually needed to improve inflammation and scarring.5
- Cut down or stop alcohol.
- Review medicines and supplements: Do not stop a prescribed medicine on your own.
- Control sugar and cholesterol: Both are linked with fatty liver. Your doctor may track them with your liver tests.
How Fast Does SGPT Come Down?
It depends on the cause. In acute viral hepatitis, ALT usually peaks around day 7 to 12 and returns to normal by week 3 to 5 if recovery goes smoothly. Fatty liver or long-term hepatitis can take months.3
Frequently Asked Questions
Is the SGPT test for fatty liver?
Not only. It checks for liver cell injury from any cause. Fatty liver is a common reason it rises. An ultrasound may show fat in the liver, but your doctor may need other tests to find the cause and check for damage or scarring.
Do I need to fast for an SGPT test?
Not always. It depends on the other tests done on the same sample and on your lab. MedlinePlus says fasting for several hours is often advised, so follow what your lab or doctor tells you.
Is SGPT 60 dangerous?
SGPT 60 U/L is above the ACG healthy range. Do not ignore it. If you feel well, book a routine appointment with your doctor to find the cause. If you have symptoms or other abnormal results, see a doctor sooner.
Can exercise raise SGPT?
Yes. StatPearls (2024) notes that extreme exercise can raise ALT for a short time to about two times the normal limit, likely from working muscles. Avoid intense exercise before the test and tell your doctor if you trained hard recently.
Can SGPT be normal with liver disease?
Yes. The 2023 AASLD guidance says liver enzymes are often normal even with advanced fatty liver scarring. So a normal SGPT alone does not rule out serious liver disease.
